As mandated by the Affordable Care Act, employers were obligated to distribute Forms 1095-B and 1095-C to all employees for the 2024 calendar year. However, under the recent Paperwork Burden Reduction Act, this requirement has been lifted, and employers are only required to provide these forms upon request.
Current and past employees can access these forms by logging into their UKG account or contacting the LSI Benefits Team or your local HR team member. Requests and/or questions can be emailed to benefits@lsius.net, mailed to 1800 Technology Drive NE, Willmar, MN 56201, or requested via phone at 320-235-8850.
